You may view the Entry Form by selecting Entry Form from the View Menu. To print the Entry Form, select Print:Entry Form from the File Menu.
Whenever you choose Entry Form from the View Menu or Print:Entry Form from the File Menu, you will be given the opportunity to edit, remove, or enter the directions and comments that will appear on the Entry Form.

Comment Paragraphs
For the Entry Form, you may add a directions paragraph which will be placed above the schedule and two comment paragraphs which will be placed after the schedule. The first paragraph is called directions only to indicate where it is placed on the Entry Form.
You can print or view the Entry Form for any week where you have entered a schedule. However, be advised that the directions and comments are not archived weekly. Only one set of directions and comments are saved with each file. You do, however, have some flexibility with them.
When you run The Mac Quarterback 1.2 and open a pool file, the directions and comments for that file are placed on the Mac QB clipboard. When you choose to view or print the Entry Form, the directions and comments that are displayed are the ones on the clipboard. Changing them changes the clipboard but does not affect the original directions and comments, unless you Save the changes.
If you change the directions and comments for a particular week, but wish to print out another week with the original directions and comments, click the Restore button.
Include Teams' Won-Lost Records
If you like, you can have the Entry Form include team won-lost records. Mac QB will automatically update team records based upon the scores you've entered. If you've check this option, Mac QB will bring up a window to allow you to edit the w-l-t records in case of error or enter w-l-t records for teams not regularly included in the pool.

The Entry Form heading has the Pool Title as the main heading and below that the Sub-Title. Beneath the Sub-Title is the Week # and the date. The Pool Title and Sub-Title are those you entered in the Weekly Pool Settings. You can change these to fit special occasions or for whatever reason. Some managers use the Sub-Title to indicate the entry fee and others to indicate the previous week's winner.
Note the entry line next to each team. Players indicate their picks on the entry lines, or in a Confidence pool, their confidence points.
窶｢ If your pool is Against the Spread, as in the example above, the underdog team has the spread points after its name. If you're running a pool with outright winners instead of against the spread and want the spread included so the players will know the favorites, you can have the spread included by entering the betting lines for the week in the Enter Betting Line item of the Pool Menu.
窶｢ The Entry Form for Confidence pools has a row of confidence point check boxes placed above the schedule of games for the number of games the player must pick. This is provided so the player can keep track of the confidence points he/she has used so that he/she is less likely to leave out a number or duplicate numbers.

They are numbered beginning with the highest confidence points down through the number of games the player must pick. In the above example, there were only 13 games that week. Since the manager has set the highest confidence points to 15 in the Confidence Template, the confidence points check off boxes begin with 15 and end with 3. If he had set the highest confidence points to a low number, the boxes would begin with 13 and end with 1.
窶｢ The Entry Form for the Post Season pool does not use a schedule but lists the playoff teams with a confidence points entry line for each. The players must enter confidence points for every team listed.

Printing Entry Forms
Multiple Entry Forms Per Page
You can have 1,2 or 4 entry forms per page by reducing the image size in Page Setup. When you choose to print entry forms, Mac QB will open a dialog window that will tell you the reduction needed for 2 per page and for 4 per page.
窶｢ If you want only one per page, leave the image size at 100.
窶｢ Its calculations are based upon the font and size you have chosen in the Format Preferences. Some fonts have such a large ascent (the area above the letter 'a' used for letters such as t and f) and descent (area below 'a' for letters such as p,g,y) that the reduction needed for 2 per page will be the same as 4 per page.
窶｢ For higher quality print, set the font and size in Format Preferences to Times 12 before printing entry forms.
窶｢ If you have chosen personalized entry forms and set the reduction to 2 or 4 per page, it will print the personalized forms, and if there is space left over on the last page, it will finish the page with regular entry forms. (No sense wasting all that space!)
Personalized Entry Forms
If you want the player's name printed on the entry form rather than Name_____, choose Print: Personalized Entry Form from the File Menu. If you want personalized entry forms for the active players only, be sure that option has been set in the Program Preferences, accessible through the Edit Menu.
Mac QB does not provide for printing a personalized Entry Form for one player only. However, you can get around this by doing the following:
1. Check the Active Players Only box in the Personalized Entry Form section of the Program Preferences.
2. Before choosing Personalized Entry Forms, pull down the Active Players menu and count down to the player you want.
3. When the standard Print Dialog box appears, enter the players number in both the beginning and ending pages boxes.
If the player you want is an inactive player, temporarily set the Inactive Weeks in the Program Preferences to 25. Now, all players will be listed under the Active Players Menu. Count down to the player and use that number for the beginning and ending pages boxes in the Print Dialog window.
